Large ovarian tumors are rare but can present with significant clinical challenges due to their size and associated complications. At Sushruta Multispecialty Hospital & Research Centre, Dr. Sangeeta Antaratani, Senior Obstetrician and Gynaecologist, successfully managed a rare and huge ovarian tumor through laparotomy in a middle-aged woman.
A 49-year-old unmarried female presented with:
Abdominal pain with vomiting for 1 week
Past history of abdominal hysterectomy 15 years ago for abnormal uterine bleeding
On physical examination, a large abdominal mass equivalent to a 30-week pregnancy was detected.
Surgical Management
Procedure: Exploratory laparotomy
Intraoperative findings: Bowel and bladder were found adherent to the cyst wall, making dissection challenging.
Surgical outcome: After a meticulous 2.5-hour surgery, a 2.5 kg ovarian mass was carefully removed.
The patient tolerated the procedure well and recovered without complications.

Discussion
Huge ovarian tumors are uncommon and can mimic advanced pregnancy due to their size. They are often associated with:
Pressure symptoms (abdominal pain, vomiting, or distension)
Surgical challenges due to adhesions with nearby organs
Successful management requires:
Careful surgical planning
Meticulous dissection to avoid damage to bowel or bladder
Expert postoperative care to ensure smooth recovery
Conclusion
This case highlights the importance of timely diagnosis and surgical expertise in managing rare, large ovarian tumors.
