A 13-year-old girl was referred to our hospital as a suspected case of hematocolpos. She presented to the outpatient department with:
Complaints of abdominal pain for the past 4 months
No previous medical or surgical history
On clinical examination and confirmation with sonography, the diagnosis of hematocolpos due to a lower third transvaginal septum was made.
Surgical Management
Findings: A lower third transvaginal septum obstructing menstrual flow was identified.
Procedure: Septal resection was performed under sterile conditions.
Outcome: Approximately 250–300 ml of old collected blood was drained from the vaginal cavity.
Post-operative care: A vaginal mold was placed to maintain patency and prevent recurrence.
The patient made an uneventful recovery and was discharged on the 3rd post-operative day in stable condition.
Hematocolpos is a rare condition resulting from congenital obstruction, leading to the accumulation of menstrual blood in the vagina. It is often caused by anomalies such as an imperforate hymen or a transverse vaginal septum.
Early recognition is vital to prevent complications such as infection, pelvic endometriosis, or infertility.
Septal resection remains the treatment of choice, with excellent prognosis when performed timely.
