Case Overview:
A 33-year-old woman presented to our hospital with 3½ months of pregnancy. During a routine antenatal scan, a complex cystic lesion was unexpectedly discovered in her left ovary. This lesion was diagnosed as an endometriotic cyst, commonly referred to as a "chocolate cyst" due to the presence of old, collected blood.

Diagnosis: Single live intrauterine pregnancy of 13 weeks with a large left ovarian cyst measuring 12 x 10 cm.
The cyst was not only large but also attached to the uterus and other internal organs, posing a significant surgical challenge and potential risk to the pregnancy.

A Rare Surgical Decision:
Given the size and complexity of the cyst, and its close association with internal structures, a multidisciplinary decision was made to perform laparoscopic surgery — a minimally invasive approach — to remove the cyst along with the affected left ovary, while ensuring the pregnancy remained undisturbed.
This type of surgery during the early second trimester of pregnancy is exceptionally rare and technically demanding due to the anatomical and hormonal changes in the body.
Postoperative Outcome:

Follow-up Scan: Single live intrauterine pregnancy of 14 weeks.
The left ovary was not seen, confirming successful removal, and the pregnancy was progressing normally postoperatively.
